Individuals looking to book an appointment to shop at Hope Chest can do so by filling out the form below. Once you fill out the form, you will receive an email back from our scheduler with your appointment information.

Individuals can also obtain a letter from a school official or minister/pastor stating the need for Hope Chest's services. This letter must be on official letterhead and can be brought to Hope Chest during open hours.
